This position collaborates closely with the Client Services Manager and other team members to develop individualized service plans and advocate on behalf of clients to ensure they receive necessary assistance. The ideal candidate is empathetic, dedicated, and experienced in working with individuals experiencing homelessness or in a related human services field. They must be committed to fostering a supportive environment and assisting clients in overcoming challenges to achieve long-term success.
Responsibilities
~1 min read- Provide day-to-day support to clients, addressing their immediate needs and concerns.
- Assist clients in accessing resources such as housing, employment, healthcare, and other community services.
- Help clients navigate the challenges of homelessness by providing emotional support and practical guidance.
- Develop and maintain positive, trusting relationships with clients to foster a safe and supportive environment.
- Collaborate with the Client Services Manager and other team members to develop and implement individualized service plans for clients.
- Monitor client progress and update case notes regularly to ensure accurate documentation of services provided.
- Coordinate with external service providers and partners to connect clients with necessary resources and support.
- Advocate on behalf of clients to ensure they receive the services and assistance they need.
- Support the implementation of Dakota Woodlands’ programs and services by facilitating client intake, assessments, and referrals.
- Participate in group activities and workshops aimed at enhancing clients’ skills and knowledge.
- Contribute to the development of new programs and services that address the evolving needs of clients.
- Maintain accurate and confidential client records in compliance with organizational policies and procedures.
- Assist with data collection and reporting for program evaluation and improvement.
- Ensure compliance with all relevant regulations, policies, and procedures related to client services.
- Work closely with other staff members to ensure a coordinated approach to client care.
- Participate in regular team meetings, training sessions, professional development, and personal care opportunities.
- Contribute to a positive team environment by sharing knowledge, experiences, and resources with colleagues.
- Perform other duties and responsibilities as assigned to support the organization's mission and client needs.
Requirements
~1 min read- Bachelor’s degree in social work, human services, psychology, or a related field; or equivalent experience in a social services role.
- Previous experience working with individuals experiencing homelessness or in a related human services field is highly preferred.
- Strong interpersonal and communication skills, with the ability to work effectively with diverse populations.
- Empathy, patience, and a genuine commitment to helping others.
- Ability to handle sensitive and confidential information with discretion.
- Strong organizational skills and attention to detail.
- Proficiency in using basic computer applications (e.g., Microsoft Office, client management software).
- Full-time position with potential for occasional evening or weekend shifts.
- Active work environment, requiring regular interaction with clients and community service providers.
