As a** Physical Therapist**, you will be a key member of our care team, responsible for assessing, planning, and delivering therapy treatments that promote independence, safety, and functional ability—primarily for geriatric or frail populations.

Key Responsibilities: ✔️ Conduct therapy assessments ✔️ Develop personalized care plans in collaboration with the interdisciplinary team (IDT) ✔️ Deliver and document therapeutic interventions ✔️ Perform treatments such as active/passive exercises, muscle re-education, gait training, and activities of daily living (ADLs) ✔️ Apply therapeutic modalities (heat, electrical stimulation, massage, etc.) ✔️ Supervise Therapy Assistants and support Functional Maintenance Programs ✔️ Educate patients, caregivers, and staff on the proper use of therapeutic equipment
