Are you a compassionate and highly skilled Nurse Practitioner passionate about providing meaningful, patient-centered care to individuals with serious illness? We are seeking an experienced Palliative Care NP/PA to join our growing team and make a profound impact in the lives of patients and families throughout Salem County, NJ.

Responsibilities
~1 min readAs an Outpatient Palliative Care Advanced Practice Provider, you will play a critical role in improving quality of life for patients facing serious or complex illness by providing compassionate symptom management, advance care planning, and care coordination.
- →Assess and manage pain and other complex symptoms in the outpatient setting
- →Lead compassionate goals-of-care and advance care planning conversations
- →Support patients and families with hospice readiness and facilitate hospice referrals when appropriate
- →Collaborate closely with physicians, specialists, home health, hospice teams, and community referral partners
- →Participate in interdisciplinary team meetings and care conferences
- →Coordinate seamless transitions across outpatient, hospital, home health, and hospice settings
- →Serve as a trusted clinical resource and advocate for patients and families
- Compassionate, empathetic, and patient-focused
- Skilled in complex conversations and relationship-building
- Comfortable working independently while collaborating within a multidisciplinary team
- Passionate about delivering exceptional palliative care and improving patient quality of life

Requirements
