Ennoble Care is a mobile primary care, palliative care, and hospice service provider with patients in New York, New Jersey, Maryland, DC, Virginia, Oklahoma, Kansas, Pennsylvania, and Georgia. Responsibilities
~1 min read- →Serve as the primary liaison between patients, families, and interdisciplinary care teams to ensure clear and consistent communication.
- →Coordinate, schedule, and track patient appointments, follow-ups, and referrals to specialists to ensure timely care delivery.
- →Process and manage referrals, including sending orders and referrals following provider appointments.
- →Maintain and manage assigned patient caseloads, ensuring all interactions are accurately documented.
- →Utilize Electronic Medical Record (EMR/EHR) systems to maintain accurate, detailed, and confidential patient records.
- →Demonstrate strong computer proficiency, including the ability to multitask and toggle between multiple systems and platforms efficiently.
- →Review provider documentation and progress notes for chronically ill patients and communicate recommendations to appropriate clinicians.
- →Develop and maintain individualized patient care plans and provide ongoing care management and coordination services using the organization’s EMR system.
- →Identify and address barriers to care, such as transportation, financial limitations, or access to services.
- →Connect patients and families with community resources, social services, and support programs.
- →Facilitate smooth transitions of care between hospitals, specialists, home health services, and other care settings.
- →Maintain frequent communication with patients and families to provide care coordination, education, and support.
- →Monitor patient adherence to care plans and support compliance with palliative care programs to improve overall health outcomes
