Responsibilities

  • Serve as the primary liaison between patients, families, and interdisciplinary care teams to ensure clear and consistent communication.
  • Coordinate, schedule, and track patient appointments, follow-ups, and referrals to specialists to ensure timely care delivery.
  • Process and manage referrals, including sending orders and referrals following provider appointments.
  • Maintain and manage assigned patient caseloads, ensuring all interactions are accurately documented.
  • Utilize Electronic Medical Record (EMR/EHR) systems to maintain accurate, detailed, and confidential patient records.
  • Demonstrate strong computer proficiency, including the ability to multitask and toggle between multiple systems and platforms efficiently.
  • Review provider documentation and progress notes for chronically ill patients and communicate recommendations to appropriate clinicians.
  • Develop and maintain individualized patient care plans and provide ongoing care management and coordination services using the organization’s EMR system.
  • Identify and address barriers to care, such as transportation, financial limitations, or access to services.
  • Connect patients and families with community resources, social services, and support programs.
  • Facilitate smooth transitions of care between hospitals, specialists, home health services, and other care settings.
  • Maintain frequent communication with patients and families to provide care coordination, education, and support.
  • Monitor patient adherence to care plans and support compliance with palliative care programs to improve overall health outcomes
