Your Role 

As a Vice President of Clinical Operations with Evergreen Nephrology, you are a senior clinical operations leader responsible for the performance, scalability, and continuous improvement of enterprise clinical programs. This role provides strategic and operational leadership across clinical programs, with accountability for clinical quality, HEDIS/Stars performance, utilization outcomes, patient engagement, and value-based care results.  

This leader will translate clinical strategy into scalable operating models, workflows, technology requirements, and performance management systems that support disciplined execution across markets and programs. The VP will partner cross-functionally to improve outcomes, reduce total cost of care, strengthen care coordination, and ensure patients receive the right care at the right time in the right setting.  

The ideal candidate is a licensed clinician and proven operator with hands-on experience leading technology-enabled clinical programs in a complex, fast-paced healthcare environment. They bring a track record of implementing and optimizing care management platforms, building high-performing teams, and driving measurable improvements in quality, utilization, engagement, and operational performance.  

This role oversees programs including CKD Navigation, Post-Acute Care optimization (PAC), Compassionate Care (ECCP) and palliative pathways, Pharmacy, and future clinical programs as the organization continues to scale.  

Role Responsibilities 

  • Own performance across CKD Navigation, PAC, ECCP, Pharmacy, and future clinical programs  
  • Ensure clinical programs align with value-based care contract requirements, quality goals, utilization targets, and enterprise clinical strategy  
  • Lead program integration, stabilization, and scaling across markets and payer partnerships  
  • Translate clinical priorities into workflows, protocols, technology requirements, reporting needs, and frontline execution  
  • Serve as the senior escalation point for program risks, performance gaps, and complex operational challenges  
  • Partner with Central Quality to translate enterprise quality strategy into program level execution, workflow design, and frontline accountability  
  • Partner with Product, Technology, Analytics, and Clinical teams to implement and optimize care management platform functionality, clinical workflows, reporting, and patient engagement capabilities  
  • Lead and advance technology enabled care delivery models that improve patient engagement, documentation accuracy, workflow efficiency, communication, and measurable outcomes  
  • Support integrated patient engagement strategies across outreach, communication, care coordination, and campaign-based workflows  
  • Drive data integrity and reporting alignment across Evergreen technology platforms and operational reporting systems  
  • Own program level KPI performance and use rapid cycle improvement to identify and close performance gaps  
  • Build and develop high performing clinical leadership teams with clear role clarity, performance expectations, and accountability structures  
  • Lead operational integration across programs to create a more coordinated patient and provider experience  
  • Other duties consistent with this role, as assigned. 

Required Qualifications 

  • Licensed clinician with experience in direct patient care and clinical operations.  
    • Backgrounds may include nursing, advanced practice providers, pharmacy, social work, or medicine  
  • Experience leading clinical operations in value-based care, provider enablement, population health, or complex chronic care models.   
  • Experience partnering with physician leaders and building strong clinical relationships.   
  • Strong working knowledge of HEDIS, Stars, quality performance, utilization management, and value-based care economics  
  • Hands on experience implementing, optimizing, or operationalizing care management platforms, clinical workflow tools, or patient engagement capabilities  
  • Experience translating operational needs into technology requirements and ensuring platform workflows support real frontline execution  
  • Experience using digital engagement, communication workflows or campaign-based outreach to improve patient engagement and program performance  
  • Ability to lead across centralized and embedded teams, multiple markets, and matrixed partners  
  • Proven ability to manage outcomes in a fast moving, evolving environment  
  • Track record of building disciplined operating systems, clear accountability, and scalable clinical programs  
  • Experience leading through influence across clinical, operational, analytics, technology, and payer partnership teams  
  • Intermediate skills with MS Office Suite of products including Outlook and Teams 
  • Able to work effectively in a primarily remote environment: 
    • Home internet must support a minimum download speed of 25 Mbps and upload speed of 10 Mbps. Cable, Fiber, or DSL connections hardwired to the internet device are recommended 
    • Evergreen will provide remote employees with telephony applications and equipment to meet the business requirements for their role 
    • Employees must work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information 

 

Preferred Qualifications 

  • Experience leading clinical operations in a Nephrology setting
  • Experience in a clinically integrated mode 

  • Bachelor's degree 

Compensation 

The pay range for this role is $200,000-$220,000 annually. Exact pay is determined based on experience, education, demand for the role, and other role-specific factors.   

This role is also eligible for an annual bonus.
