Manages primary caseload of Hospice patients per physician's instructions and in compliance with regulations.

Implements and updates care plans to address patient's nursing needs.

Educates and supports patients, family, and staff on nursing care and patient needs.

Collaborates with the Interdisciplinary Team (IDT) to meet all patient and family care requirements.

Supervises Licensed Practical Nurses and Hospice Aides.

Certified Hospice and Palliative Nurse (CHPN), preferred.

2+ years' experience in hospice or palliative care but willing to train the right person.

Ability to collaborate within an interdisciplinary setting.

Knowledge and understanding of hospice philosophy and principles of death/dying.

Valid driver's license and reliable, insured transportation in accordance with New Jersey state requirements.

Travel required.
