We are seeking a dedicated and compassionate Nurse Practitioner to join our team at Fellowship Village. As a Nurse Practitioner, you will play a crucial role in providing primary healthcare services to our residents, promoting health and wellness, and ensuring the highest standards of care.


Job Type: Full-time
Pay: From $75.00 per hour

  • Conduct comprehensive health assessments providing preventive and primary care for Independent Living residents.
  • Develop and implement individualized care plans in collaboration with residents, families, and interdisciplinary teams.
  • Manage and treat acute and chronic medical conditions, including but not limited to hypertension, diabetes, and respiratory illnesses.
  • Perform routine check-ups and follow-up appointments to monitor residents' health status and adjust care plans as needed.
  • Provide education and support to residents and their families on preventive care, medication management, and healthy lifestyle choices.
  • Support the department's utilization review process.
  • State certification by the New Jersey Board of Nursing as an Advanced Nurse Practitioner.
  • State of New Jersey registered professional nurse license.
  • Seasonal flu vaccine required.
  • National certification in adult and/or geriatric medicine, a plus.
  • Experience in adult/geriatric medicine.
  • Excellent communication and interpersonal abilities.
  • Ability to work effectively as part of a multidisciplinary team.
  • Compassionate and patient-centered approach to care.
  • CPR and BLS certifications.
