We're seeking a part-time passionate BC/BE Internal Medicine physician to join our team of experienced practitioners and specialists delivering exceptional patient care to older adults.

Schedule: Mondays 4 to 5 hours at Applewood Village and Tuesdays, 5 to 6 hours at Pines Village

Provide comprehensive medical care and management for sub-acute residents, with occasional support for Assisted Living (AL), Long-Term Care (LTC), and Independent Living (IL) residents as needed.

Conduct thorough geriatric assessments to evaluate physical, cognitive, functional, and other aspects of health, tailoring care plans to meet individual needs.

Manage complex medical conditions commonly associated with aging, ensuring patient safety, comfort, and quality of life.

Collaborate with interdisciplinary teams, including Nurse Practitioners, nurses, and allied health professionals, to develop and implement personalized care plans.

Communicate proactively with families, addressing concerns and providing updates on patient progress.

Educate patients, families, and caregivers on health issues, disease management strategies, medication management, and preventive health measures.

Comply with regulatory requirements, documentation standards, and ethical principles specific to the nursing home environment.

Handle admissions and quarterly reviews as part of ongoing patient management.

MD with a valid medical license in New Jersey (Board certification or eligibility in Internal Medicine required).

2+ years of relevant experience in primary care or geriatric medicine.

Demonstrated excellence in bedside manner, fostering positive interactions with patients, families, and staff.

Strong clinical and communication skills, with the ability to work collaboratively within an interdisciplinary team.

Familiarity with the unique environment and regulations of nursing homes.

Local travel may be required.
