Job Title: Community Outreach Physician Liaison
Location: Westminster, CO
Compensation: $68-72k/year
The Community Outreach Physician Liaison integrates patient engagement, community outreach, and physician relationship management to enhance patient retention, increase intakes, and strengthen Giving Home Health Care’s (GHHC) regional presence. This role combines proactive patient and caregiver support with strategic physician collaboration to reduce transfers out, grow referral networks, and ensure seamless care transitions. The Community Outreach Physician Liaison serves as a bridge between patients, community stakeholders, and medical providers, driving organizational goals through personalized outreach, physician education, and data-driven performance.
Compensation & Perks
· Base Salary: $68-72k/year
· Mileage Reimbursement
· $100/month cell phone reimbursement
