We are now hiring for the Patient Coordinator role in our Westminster, CO location! The Patient Coordinator is vital in ensuring effective communication and support between our healthcare team and patients.   • The Patient Coordinator is responsible for enhancing the overall patient experience and ensuring that patients receive the necessary support and resources. 
  • Accompany patients to their benefit renewal appointments to advocate for the patient and the program.
  • Visit patient homes and update patient care. 
  • Effectively communicate and provide information to patients, caregivers, and physicians. 
  • Understand HIPAA and maintain confidentiality.
  • Display a warm and friendly attitude.
  • 1 to 3 years of experience in a Care Team or Patient Navigation Role preferred
  • Healthcare experience preferred
  • Key competencies include, empathy, problem solving, communication, active listing, and tenacity
  • Self-motivated with the ability to manage time appropriately.
  • Experience with MS Office products (Outlook, Word, Excel, etc.)
  • Minimum associate level education preferred. (High school diploma or equivalent is required.) 
  • 1 year of medical knowledge preferred.
