Patient Coordinator
Location: Amarillo, TX
Compensation: $23-25/hour
 
As a Patient Coordinator, you’ll be a key advocate and support system for our patients and care teams. You’ll help ensure patients receive the care, resources, and attention they deserve—making a meaningful impact every day.
 
What You’ll Do
Patient Support
· Receive patients through a warm handoff from Patient Development team
· Attend doctor visits to support and facilitate communication
· Conduct home visits to assess needs and deliver resources
· Perform initial setup visits for newly approved patients
· Assist with caregiver onboarding and program education
 
Communication
· Be the main point of contact for ALL patient inquiries and updates
· Facilitate communication between all departments to ensure coordinated, patient-centered care
· Make courtesy calls to check on well-being and address concerns
· Conduct milestone calls to track progress and share timely updates
 
Advocacy
· Collaborate with providers to ensure personalized, high-quality care
· Advocate for patient needs and rights within the organization
  • Healthcare experience (preferred)
  • Familiarity with MS Office (Outlook, Word, Excel)
  • Excellent communication, empathy, and problem-solving skills
  • Ability to manage time independently and meet deadlines
  • Reliable transportation for home visits and appointments
  • Exceptional organizational skills and attention to detail
  • HS diploma or equivalent required
