About the Role
~1 min readThe Executive Director of Home Health & Hospice provides executive leadership and full operational accountability for a multi-service home health and hospice organization operating in a non-profit environment. This role is responsible for driving sustainable growth, maximizing profitability, ensuring regulatory compliance, and scaling operations efficiently while maintaining high standards of clinical quality and patient experience.
The Executive Director serves as the senior leader responsible for aligning clinical excellence with business performance—overseeing strategy, operations, financial outcomes, referral development, and talent management across both service lines.
Strategic Growth & Market Expansion
- Develop and execute strategic growth plans that increase admissions, census, and market share across home health and hospice service lines.
- Identify and pursue new growth opportunities including geographic expansion, service diversification, payer strategies, and referral channel optimization.
- Analyze market trends, competitor activity, and referral patterns to guide expansion and investment decisions.
- Partner with corporate leadership to align local growth initiatives with enterprise-wide strategic objectives.
Profitability & Financial Performance
- Own full P&L responsibility, ensuring achievement of revenue and margin
- Lead budgeting, forecasting, and financial performance monitoring, with a strong focus on labor management, visit productivity, cost controls, and revenue optimization.
- Drive disciplined financial management across staffing models, overtime, contract labor, and supply utilization.
- Partner closely with revenue cycle teams to ensure accurate documentation, billing, collections, and denial management.
- Translate financial data into actionable operational strategies to improve efficiency and profitability.
Efficient Referral Development & Relationship Management
- Lead the development and execution of a high-performance referral strategy focused on consistent, scalable admission growth.
- Build and maintain strong relationships with hospitals, physicians, SNFs, assisted living communities, payers, and community partners.
- Ensure referral intake, admission processes, and handoffs are efficient, responsive, and customer-focused.
- Monitor referral conversion rates, admission cycle times, and payer mix to drive optimal performance.
Operational Scalability & Performance Optimization
- Design and implement scalable operational models that support growth without sacrificing quality, compliance, or staff engagement.
- Standardize workflows, policies, and performance expectations across teams to enable consistency and efficiency.
- Leverage data, dashboards, and key performance indicators (KPIs) to monitor operational performance and proactively address risks.
- Optimize staffing models to support fluctuating census levels while maintaining productivity and employee satisfaction.
- Lead continuous performance improvement initiatives to reduce variation, improve throughput, and enhance patient outcomes.
Clinical Quality & Regulatory Compliance
- Ensure full compliance with all federal, state, and local regulations, including CMS Conditions of Participation for Home Health and Hospice.
- Oversee clinical leadership to ensure delivery of high-quality, patient-centered care and strong survey readiness.
- Ensure quality assurance and performance improvement (QAPI) initiatives are appropriate to improve outcomes, star ratings, and patient satisfaction.
- Foster a culture of accountability, safety, and ethical practice across all levels of the organization.
Leadership, Culture & Talent Development
- Partner with Talent Acquisition to recruit, develop, and retain high-performing clinical and operational leaders.
- Establish clear expectations, performance goals, and accountability frameworks for leadership and frontline teams.
- Promote a culture that balances compassionate care delivery with operational excellence and financial stewardship.
- Lead change management initiatives associated with growth, restructuring, or operational transformation.
External Representation & Stakeholder Engagement
- Serve as the senior representative of the organization in the community and with key external stakeholders.
- Partner with payers, vendors, and strategic partners to support growth and operational effectiveness.
- Represent the organization in professional, regulatory, and industry forums as appropriate.
Requirements
~1 min read- Bachelor’s degree required; Master’s degree in Healthcare Administration, Business Administration, Nursing, or related field preferred.
- Active RN license strongly preferred.
- Minimum of 5–10 years of progressive leadership experience in home health, hospice, or post-acute healthcare operations.
- Demonstrated success leading growth initiatives, managing full P&L responsibility, and scaling healthcare operations.
- Strong working knowledge of Medicare, Medicaid, managed care, and hospice reimbursement models.
