Home Care RN Supervisor

The Home Care RN Supervisor will support the Executive Director Home Care & GLAH in the day-to-day operations of Goodwin House Home Care including daily assessment to initiate care, managing phone calls from clients and families, working in partnership with our scheduling team to ensure clients get the care coverage requested and a ending multidisciplinary review meetings.  In addition, the RN Supervisor has personnel responsibility that includes managing disciplinary actions, performance reviews, education and training.

The pay for this role is $50/hr-$52/hr

• Responsible for the initial assessment, reassessment, enrollment, and development of the plan of care and coordination of GH Home Care Services.
• Evaluate all requests for incoming Home Care referrals to determine home care services needed/required/requested.
• Coordinate with Home Care clinical and office staff to schedule and fill needed services. 
• Provides direct supervision to all Certified Nursing Assistants and Companion staff.
• Assist in the oversight of newly hired Home Care Assistants and Companions.
• Assist in the planning and implementation of in-service (e.g., skills fair) and continuing education programs to meet education and training needs required.
• Assist in the coordination and oversight of all direct and indirect resident services provided by GH Home Care staff to include RN supervisory visits, resident satisfaction (i.e., complaint management), assessment, re-assessment, and updating the resident plan of care. 
• Responsible for keeping the Executive Director Home Care & GLAH informed of any changes in workload demands. 
• Review and complete clinical record documentation to ensure compliance with regulatory standards and adhere to designated policy and procedures.
• Assist in the ongoing evaluation of home care services and quality assurance for effectiveness and compliance with regulatory authorities. 
• Identifies/tracks/manages any identified risk management issues/resident service demands/outcomes/effectiveness of quality program. 
• Assist in the education of staff to the quality program. 
• Assists with the evaluation of organization performance via performance improvement program, productivity, quarterly and annual reviews.

• Promote and assist in the accountability of all staff to provide exceptional service especially by the front-line staff and scheduling coordinators.
• Assist in coaching and mentoring home care assistant to be compassionate – motivated and have the heart to provide exceptional care and service to resident.
• Assist in the linkage of resident/family satisfaction to employee satisfaction; serve as a liaison to meet resident choices and preferences and responsiveness of scheduled service.
• Model Goodwin Living organization mission and values by demonstrating the responsibility for operational efficiency, empowering staff to listen to opportunities to meet and exceed performance standards.
• Facilitate appropriate departmental delegation within the scope and expertise of Home Care team members.

• A Registered Nurse currently licensed in the Commonwealth of Virginia or a compact State.
• At least one year of training and experience in direct health care service delivery with at least one year within the last five years of supervisory or administrative management experience in home care or a related health program.
• Working knowledge of state, federal and local regulations, rules, and guidelines.
• Must demonstrate skill to develop administrative policies and procedures and design programs for implementation and compliance.
• Must demonstrate skill in scheduling, organizing, leadership, quality improvement and delegation.  Must be able effectively communicate in understandable English.
• Computer skills required.
• CPR certified.
• Must be able to function independently, have flexibility, personal integrity and the ability to work effectively with clients, leaders and staff.
• Ability to motivate and lead a team.

• Projects a positive professional image.
