Compensation: Paid per visit
Duties Include:
Complete comprehensive initial and ongoing nursing assessments.
Perform supervisory visits of home health aides to ensure care plans are being followed.
Develop and evaluate client care plans; revising as needed for unmet goals and changes in
condition.
Maintain accurate, timely electronic medical records (EMR) for each visit to comply with
billing and regulations.
Instruct patients and caregivers on disease management, safety improvements, and self-
care techniques to promote independence.
Communicate with family, caregiver and the team any changes in the patient' s physical or
emotional condition.
Participates in medical record audit as assigned.
