ComForCare Home Health Care (North San Diego) is looking for a compassionate and skilled LVN to join our dedicated home health team in San Diego, CA. Responsibilities:

Provide skilled nursing care to home health patients in accordance with physician-approved care plans
Perform wound care, medication administration, injections, and other clinical procedures
Monitor and document patient health status, progress, and changes in condition
Educate patients and family members on disease management, medications, and home care techniques
Coordinate with RNs, physicians, and interdisciplinary care teams to ensure continuity of care
Maintain accurate and timely clinical documentation in compliance with state and federal regulations
Conduct home visits across the North San Diego service area on a scheduled basis

Requirements:

Valid California LVN license in good standing
Current CPR/BLS certification
Prior home health, skilled nursing, or clinical experience preferred
Strong knowledge of wound care, medication management, and patient assessment
Excellent communication, documentation, and organizational skills
Reliable transportation, valid driver's license, and auto insurance
Compassionate, self-motivated, and able to work independently in a field-based setting
