Extended Family Home Care Services is a premier home care agency and W-2 employer seeking PRN Home Health Aides and CNAs to perform the agency’s patient services in its service areas which span all of Southwest Florida. Lots of open hours available due to rapid growth!! PRN staff are eligible to participate in the 401(k)plan 30 days after hire. Applicants will be screened for their experience and certifications to determine which position they qualify for. Schedule

To accommodate the scheduling needs of our clients, Extended Family Home Care Services employees with PRN status must meet the minimum requirements
 
Be available for 10 weekday (Monday – Friday) shifts each month

Be available for 2 weekend (Saturday - Sunday) shifts each month
 
Be available to cover 3 holidays each year
 
Day shifts and overnight shifts available
 
Flexible hours
 
On call opportunities
 

Position Duties & Responsibilities

Provides personal care and related services in the home, under the direction, instruction, and supervision of the Supervising Nurse.
 
Tasks to be performed by an HHA must be assigned by and performed under the supervision of an RN who will be responsible for the client care/services provided by the HHA.
 
Under no circumstances may an HHA be assigned to receive or reduce any intravenous procedures, or any other sterile or invasive procedures, other than rectal temperatures or enemas.
 
Follows the plan of care to provide, safe, competent care/service to the client
 
Helps the client to maintain good personal hygiene and assists in maintaining a healthful, safe environment
 
Plans and prepares nutritious meals
 
Assists the client with ambulation as approved
 
Encourages the client to become as independent as possible according to the care plan
 
Attempts to promote client's mental alertness through involvement in activities of interest
 
Gives simple emotional and psychological support to the client and other members of the household and establishes a relationship with client and family which transmit trust and confidentiality
 
Reports any change in the client's mental or physical condition or in the home situation to the supervisor
 
Performs routine housekeeping tasks as related to a safe and comfortable environment for the client
 
Prepares a visit report promptly and incorporates same in the client record weekly
 
Confirms on a weekly basis, the scheduling of visits so that other necessary visits by staff members can be coordinated
 
Works with personnel of other community agencies involved in the client's care
 
Attends in-service as required by regulation
 
Confer with the Supervisor about client’s status and care, participate in the planning of service
 
Promptly report any change in client condition to the Supervisor

Works within the Home Aide’s scope of practice, as directed by a Registered Nurse and report in writing to the Supervising Nurse


Qualifications
 
High school diploma or equivalent

Certified Nursing Assistant or Certified Home Health Aide
 
Active AHCA Fingerprints (Level II Clearance) with Eligible status
 
CPR/First Aid Card by American Heart Association, American Red Cross, or the National Safety Council
 
Recent TB Test Result (within 12 months) or Chest X-Ray (within 5 years)
 
State of Florida Driver's License with insured vehicle
 
The ability to drive and the ability to access clients’ homes, which may not be routinely wheelchair accessible, are required.
 
Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.
 
On occasion, may be required to bend, stoop, reach and move client weight up to 250 pounds; lift and/or carry up to 30 pounds.
 
Must be able to communicate in English clearly, both verbally and in writing.
 
Must be free from health problems that may be injurious to client, self and co-workers and must present appropriate evidence to substantiate this
 
Must comprehend the basics of personal care, housekeeping and meal preparation
 
Must understand and respect client’s including ethics and confidentiality of care
