We’re seeking a compassionate and skilled In-Home Physical Therapist (PT)to provide high-quality, patient-centered care in the comfort of patients’ homes throughout Springfield,  MA and surrounding areas.

Key Responsibilities for Physical Therapist (PT):

  • Conduct thorough initial evaluations and develop personalized treatment plans tailored to each patient’s needs.
  • Provide skilled physical therapy interventions to improve strength, balance, mobility, and functional independence.
  • Educate patients and caregivers on exercises, safety techniques, and home modifications to prevent falls or injury.
  • Monitor patient progress and modify treatment plans as needed to achieve goals.
  • Document all evaluations, treatments, and progress notes accurately and in compliance with agency and regulatory standards.
  • Collaborate with physicians, nurses, and other care team members to ensure seamless continuity of care.
  • Provide professional guidance and support to patients and their families to encourage active participation in care.
  • Maintain equipment, supplies, and records necessary for home therapy sessions.

Qualifications for Physical Therapist (PT):

  • Licensed Physical Therapist (PT) in Massachusetts.
  • Previous experience in home health or outpatient physical therapy preferred.
  • Strong assessment, communication, and problem-solving skills.
  • Ability to work independently and manage a flexible schedule.
  • Reliable transportation and valid driver’s license.
