Key Responsibilities

Launch new cases and model best‑practice care
Provide hands‑on personal care and ADL support
Complete home‑safety checks and quality‑assurance visits
Train, coach, and onboard caregivers
Participate in on‑call rotation and fill urgent shifts
Problem‑solve changes in client condition
Assist with delivering supplies and documentation to field staff

What You Bring

At least 2 years of experience supporting individuals with functional disabilities and a current CNA certificate (leadership experience a plus)
Strong communication and interpersonal skills
Reliable transportation and the ability to lift/transfer clients
Comfort using mobile apps/EMR systems
Compassionate, dependable, and calm under pressure
Valid driver’s license and auto insurance
Reliable vehicle in good working order
Ability to tolerate a variety of home environments
Ability to manage physical demands, including standing or sitting for extended periods
Willingness to work every other weekend and share on‑call responsibilities
