The Community Healthcaring KW provides a broad range of services aimed at improving the health of individuals and the wider community. Our services include medical and dental care, health promotion, illness prevention and outreach programming. Our expertise and resources are focused on working with those who experience health inequities and barriers to accessing health services. Collaborating as an interdisciplinary team, we provide services and supports that address the social determinants of health including factors and barriers such as poverty, inadequate housing, food insecurity, substance use, language and culture, as well as stigma and social exclusion.

 

The Community Healthcaring KW works with the most complex client population served by primary health care professionals within Waterloo Region. Our highly skilled and diverse team works from a strength-based approach with individual clients as well as the broader community. We collaborate with many partner agencies and organizations within Waterloo Region. Our team consists of Family Physicians, Nurse Practitioners, Registered Nurses, Social Workers, Dietitians, Chiropodists, Pharmacists, Psychiatrists, Health Promoters, Community Health Workers, Administrative Staff and Volunteers.

Job Title: Nurse Practitioner - Refugee Health 

Full Time, Permanent - 1.0 FTE – 37.5 hours per week  

Salary: $117,923.72 - $138,547.50 per annum

Job Summary

The Refugee Health Nurse Practitioner role emphasizes comprehensive primary care, health promotion and disease prevention throughout the life span; focusing on our refugee and newcomer patients.  As a member of our skilled, committed team, you will practice in a multifaceted role that includes clinical practice, education and consultation.


Responsibilities



  1. Perform assessment, diagnosis, treatment, health education/counselling and other clinical activities in accordance with the College of Nurses of Ontario standards of practice for Registered Nurses in the Extended Class (NPs) and all relevant provincial legislation and regulations.
  2. Work from a Social Determinants of Health perspective to improve health outcomes for individual clients and/or for the development of community health and well-being, and to reduce health inequities.
  3. Provide comprehensive primary care services to individuals, families, and communities across the lifespan, with an emphasis on health promotion, patient education, and disease prevention.
  4. Triage client needs either over phone, or in person & maintain
  5. Maintain accurate and up to date clinical notes in accordance with NP’s professional standards and organizational policy.
  6. Participate in the development, implementation, monitoring and evaluation of health promotion/disease prevention programs for individuals and client populations as determined by the Health Centre.
  7. Work during both regular and extended hours (evenings/weekends). A minimum of one evening per week will be required.
  8. Participate in team and staff meetings and other meetings and committee work as appropriate to support both service delivery and organizational goals.
  9. Work individually and collaboratively in the development and implementation of quality improvement initiatives and activities.
  10. Work in a manner that preserves privacy and confidentiality.
  11. Work in a manner that ensures client safety and minimizes risk to clients, volunteers and the Health Centre.
  12. Work in a manner consistent with the Health Centre’s Vision, Mission and Values.
  13. Work in a manner consistent with the Health Centre’s policy and commitment to Diversity, Equity and Inclusion in all processes and practices.
  14. Work in a manner that demonstrates self-reflection and personal accountability for work performance.
  15. Contribute to the Health Centre’s practices of hiring, orienting, and training of staff.
  16. Collect, analyze, and report on data and relevant information as required.
  17. Participate in research as determined by the Health Centre.
  18. Adhere to all Health Centre policies.
  19. Adhere to appropriate infection control practices.
  20. Perform any other duties as assigned by your supervisor.

 

Accountability

The position is accountable to the Manager of Primary Care

 

Qualifications


  1. Completion of a recognized Primary Care Nurse Practitioner program.
  2. Master’s degree in nursing; or equivalent recognized education and experience.
  3. Current certification with the College of Nurses of Ontario-Extended Class and a member in good standing.
  4. 2 years minimum experience as a Primary Care NP working with all ages in a community health setting.
  5. Valid Ontario Drivers License.
  6. Knowledge of the health delivery system, community and social services and the relevant legislation related to Nurse Practitioners in Ontario.
  7. Proven experience working in a collaborative team environment; and
  8. Demonstrated leadership in promoting a shared responsibility for professional practice and development.
  9. Knowledge of cultural diversity as well as newcomer and refugee health in Canada is desirable.
  10. Ability to communicate in another language is an asset.
  11. Experience working effectively with culturally and economically diverse
  12. Ability to obtain a Police Vulnerable Sector Check satisfactory to the Health Centre.
  13. 2-Factor Authentication for Outlook is App based (Microsoft Authenticator Application) and used to ensure the security of our online accounts. 2-FA requires the use of a personal mobile device with a recent operating system.

 

Hours: 

Monday, Tuesday, Thursday: 9:00am- 5:00pm 

Wednesday 9:00am- 5:00pm or 12:00pm- 8:00pm 

Friday 8:00am - 4:00pm
