What You’ll Do: • Complete Start of Care (SOC) admissions in patients’ homes. • Conduct detailed clinical assessments and OASIS documentation. • Initiate care plans and ensure a smooth transition into services. • Educate patients and families on what to expect moving forward. • Coordinate with the care team to set patients up for success. What You Bring: • Active Minnesota RN license. • Experience with home health admissions and OASIS preferred. • Strong time management and independent work style. • Comfortable working in a field-based, patient-centered role. • Reliable transportation.
