What You’ll Do: • Evaluate patients in their home to identify barriers to independence. • Build personalized plans focused on ADLs, safety, and functional mobility. • Provide hands-on therapy in a setting that directly translates to daily life. • Recommend home modifications and adaptive equipment that make a real difference. • Educate patients and families to support long-term success. • Document care in EMR and collaborate with a supportive interdisciplinary team. What You Bring: • Active Minnesota OTR/L license. • A patient-first mindset and passion for functional outcomes. • Ability to work independently and manage your own schedule. • Reliable transportation for home visits. • Experience in home health or desire to transition into it.
