What You’ll Do • Provide skilled nursing visits in homes and community settings. • Perform assessments, wound care, and medication management. • Educate patients and families on care plans and recovery. • Adapt quickly to new territories and patient needs. • Collaborate with RN Case Managers and interdisciplinary teams. • Maintain accurate, timely documentation. What We’re Looking For • Active Minnesota Registered Nurse (RN) license. • Strong clinical judgment and willingness to learn. • Comfortable working independently in a field-based role. • Adaptable and confident in new environments. • Reliable transportation.
