Position Title: Intern – Patient Transition Integrated Care Management (Flat Rate)
Department: Integrated Care Management
Location: Riley Hospital, Indianapolis, IN
Shift: Full-Time
 
Position Overview: The Intern supports the case management team by providing client services and follow-up as needed. This position aids individuals experiencing personal and environmental challenges that may predispose them to illness or interfere with optimal healthcare outcomes. The Intern advocates for patients by securing necessary community and financial resources through extensive knowledge of human resource agencies, aiming to facilitate a smooth transition of care and support.

 
Key Responsibilities:  
Assist the case management team by providing client services and follow-up support.  
Advocate for patients by connecting them with appropriate community and financial resources.  
Support individuals facing personal or environmental difficulties impacting their health or access to care.  
Apply knowledge of various human resource agencies and community resources to aid patient needs.  
Maintain effective communication with patients, families, and healthcare team members.  
Function effectively within a multidisciplinary team environment.

 
Qualifications & Requirements:  
Graduate of an accredited MSW program, no more than 120 days post-graduation.  
Bachelor’s degree in Social Work or a related field.  
Good written and verbal communication skills.  
Knowledge of families, poverty, and diverse cultures.  
Extensive knowledge of community resources.  
Ability to function effectively within a multidisciplinary setting.
