Under pharmacist supervision, supports the pharmacy department and medical practices in ensuring adherence to medications and working with payer partners to help with pharmacy related metrics. Coordinates with pharmacists and clinical team members system-wide, pharmacies as well as external healthcare partners. Key job duties may consist of patient chart reviews, telephonic outreach and communication, medical record documentation, and data collection and management. Works with patients, pharmacies and clinical teams to resolve barriers to medication access. Exercises sound judgement, anticipates potential issues and challenges in common situations and applies standard policies and procedures to determine solutions or escalation.

  • Maintain confidentiality and uphold HIPAA standards in all communications and documentation
  • Prioritize workload effectively to meet volume demands while maintaining a high level of accuracy
  • Communicate professionally with patients, providers, and pharmacies 

 

  • Set schedule with no nights, weekends, or holidays
  • After an initial training period, partial remote work opportunities may be available based on performance, accuracy, and departmental metrics
