As the Nurse Navigator, you will play a vital role in coordinating the care for our patients, developing personalized care plans in collaboration with patients and their primary care providers. Your responsibilities will include:
Collaborating with physicians, nurses, and other healthcare professionals to ensure timely delivery of tests and treatments
Advocating for patients and balancing their care needs with financial considerations
Educating patients about the pain program; provides scheduled education to patients on a multitude of factors that affect their chronic pain.
Triaging referred patients.
Managing care delivery across the patient care continuum for new patients
Working with DME processing and managing of orders.
Collaborating with and facilitating services to meet the needs of the Limb Loss Clinic.
Requirements

Bachelors of Nursing (BSN) preferred.
Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
Requires that the RN has graduated from a nationally accredited nursing program. 3-5 years of experience preferred.
Requires Basic Life Support certification through the AHA.
Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
Specialty certifications may be required per unit/department specialty in accordance with the department provision of care.
Requires knowledge of personal computers using various types of word processing and database software.
Requires clinical work experience, medical terminology.
Requires understanding of medical record requirements, regulations and policies.
Experience with education and navigating complex conditions preferred.
