Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois. We provide exceptional serious illness care, hospice care and grief support for adults and children.
The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices.
Admission RN
We are currently seeking a part time Admission RN to assist our patients and families through the admission process. The Admission RN evaluates patient for hospice, educates patient and family, orders medications and DME, and starts the plan of care. This position is (3) 12-hour shifts with rotating weekends.
Key Responsibilities:
- Complete an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
- Provide a complete physical assessment and history of current and previous illness(es).
- Identify patient and family needs on admission and initiates Plan of Care activities. Proceeds with dissemination of information to On-Call Nurse or Case Manager.
- Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions (Includes the patient and the family in the planning process).
- Counsel the patient and family in meeting nursing and related needs.
- Provides health care instructions to the patient as appropriate per assessment and plan.
Education/Experience:
- Registered nurse with current licensure to practice professional nursing in the state.
- Minimum of two years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
