Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois.  We provide exceptional serious illness care, hospice care and grief support for adults and children.

 

The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility.  We also have a 24-bed inpatient unit in Joliet.  Non-clinical staff are based out of our Joliet administrative offices.


Nurse Practitioner:

The Nurse Practitioner provides community-based palliative care, focusing on symptom management, care planning, and coordination aligned with patient goals. This role supports quality of life, reduces hospitalizations, and assists hospice with recertification visits as needed.

 

Key Responsibilities:

  • Provide direct palliative care, including comprehensive assessments, symptom management, and care planning in community settings
  • Conduct history and physical exams, order and interpret diagnostics, and document findings accurately
  • Develop and implement individualized care plans aligned with patient goals, values, and quality-of-life needs
  • Collaborate with interdisciplinary teams, physicians, and caregivers to ensure coordinated and holistic care
  • Facilitate advance care planning discussions, including goals of care and end-of-life decisions
  • Support hospice services through face-to-face recertification visits and ensure compliance with regulatory requirements
  • Work a schedule of four (4) 10-hour days per week
  • Participate in on-call rotation one week every six weeks (telephone only)
  • No in-person visits required during on-call periods


Education/Experience:

  • Current RN license
  • Graduate of accredited nursing program
  • Master’s degree with 1+ year NP experience and 1+ year in hospice/palliative care
  • State-required advanced practice nursing certification
  • Specialty certification preferred (CHPN, Pediatrics, or Geriatrics)
  • Valid driver’s license and insured, reliable vehicle
