The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices.
RN Field Case Manager:
The Field Registered Nurse (RN) Case Manager is responsible, as a member of the interdisciplinary team, for the provision of skilled nursing assessment and hospice care to maximize the comfort and health of patients and families/caregivers consistent with Lightways Hospice and Serious Illness Care policies and procedures. The RN plans and directs services to meet the needs of individuals and families/caregivers within their homes and communities.
Key Responsibilities:
- Deliver skilled nursing care and hospice services in home and community settings
- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications and treatments while monitoring patient symptoms and outcomes
- Educate patients and caregivers on care, self-management, and disease progression
- Collaborate with the interdisciplinary team to ensure coordinated, high-quality care
- Maintain accurate documentation and participate in on-call coverage as needed
- Cover DuPage and Southern Cook County (Tinley Park, Orland Park, Downers Grove, Lemont)
- Monday–Friday schedule
- Minimum of 20 on-call hours per month (may increase), supporting entire service area
Education/Experience:
- Current RN license
- Graduate of accredited nursing program
- Minimum of 2 years of nursing experience, with at least 1 year in public health, home care, or hospice preferred.
- Valid driver’s license and insured, reliable vehicle
