Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in Illinois. We provide exceptional serious illness care, hospice care and grief support for adults and children.
The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices.
Social Worker- Pediatrics
The Pediatric Social Worker provides short-term crisis intervention and individual or family counseling and support as determined by the social worker’s assessment, family input, and the IDT plan of care. Also develops and facilitates relationships with referral sources and provides information to patients and families/caregivers as appropriate. The territory for this position is I88 and south. This is a Field position travelling to patients homes, nursing homes or wherever the patient calls home. There is a requirement to be in the office two times a month for team meetings.
Responsibilities:
- Assesses each patient/family’s unique social and emotional needs as indicated.
- Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required.
- Carries out social evaluations and plans intervention based on evaluation findings.
- Counsels patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties.
- Develop and facilitate relationships with referral sources.
- Participates in covering on-call after normal business hours/weekends based on patient and organizational needs.
Education/Experience:
- Master’s in Social Work from an accredited graduate program required.
- LSW required; LCSW desirable.
- Minimum of one year's experience in health care, hospice experience preferred.
