Lead Options Counselor – social service support coordination

Liberty Healthcare is hiring for a full-time Lead Options Counselor position.

As a Lead Options Counselor, you will be an active contributor with a statewide Aging and Disability Resource Center system which supports older individuals and people who are living with disabilities.

• Functioning as the “go to” resource for people who are seeking information on long term care and other community-based services and supports

• Providing person-centered planning in a way that supports informed decision making and the ability to secure services which will enable people to live as independently as possible

• →Leading a team of six field-based Options Counselors

• →Ensuring a significant number of Kansans receive high quality options counseling services

• →Data analysis and reporting

• →Participating in quality assurance initiatives

• →Providing guidance and consultation on complex matters

• →Meeting with participants and their families in-person and virtually

• →Assessing participant’s needs, providing information, and answering questions

• →Connecting people with appropriate services and supports

• →Documenting interactions, services, and relevant information in computer systems

• →Traveling to communities throughout Kansas and participating in onsite meetings

• →Creating partnerships with stakeholders

This position will require you to travel to communities throughout Kansas for in-person meetings and allow you to partially work from your home office.

• Salary starting at $60,000
