Responsibilities

  • Assess Client Needs: Evaluate clients' physical, emotional, and social needs.
  • Develop Care Plans: Create and adjust individualized care plans.
  • Provide Support and Counselling: Offer emotional support, counselling, and guidance.
  • Coordinate Services: Collaborate with healthcare providers and community organizations.
  • Advocate for Clients: Ensure clients receive necessary services and support.
  • Maintain Records: Keep accurate records of client interactions and progress.
  • Crisis Intervention: Provide immediate support in crisis situations.
  • Participate in Meetings: Attend and contribute to multidisciplinary team meetings.

Requirements

  • Education: Bachelor's or master’s degree in social work.
  • Licensure: SWE registration.
  • Experience: Previous experience working with adults in social work or related setting preferred.
  • Skills: Strong communication, interpersonal, and organizational skills. Ability to work independently and as part of a team. Empathy, patience, and resilience in challenging situations.
  • Requirements: Social Work England registration, eligibility to work in the UK, qualification in Social Work at degree level, and post-qualified experience.
