Social Worker – Community Mental Health Team – Rochdale

Liquid Personnel is currently recruiting for a Social Worker to join a Community Mental Health Team based in Rochdale.

What will your responsibilities be?

• Undertake statutory adult social care functions within a secondary mental health service, working with individuals experiencing severe and enduring mental ill-health.
• Complete strengths-based assessments, scheduled and unscheduled reviews, care planning, and commissioning of support in line with individual needs.
• Carry out Mental Capacity Assessments, Section 42 safeguarding enquiries, and facilitate timely hospital discharges in collaboration with health colleagues.
• Apply sound knowledge of key legislation, including the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983, and associated frameworks such as DoLS, CHC, and data protection regulations.
• Work in a co-located, multi-disciplinary team setting across the Mental Health Review Team (Middleton) and CMHT West (Rochdale), supporting service users both within and outside of borough placements.

• SWE registration  
• Eligible to work in the UK  
• Hold a full UK license  
• Post-qualified experience  
• Degree level or equivalent in Social Work
