Liquid Personnel is recruiting a Social Worker to join its client’s Adults Mental Health Acute Discharge team based in Stoke.

As a Mental Health Social Worker involved in Section 117 aftercare discharges, your primary responsibility is to ensure that individuals who have been detained under certain sections of the Mental Health Act (typically Sections 3, 37, 47, or 48) receive appropriate aftercare services upon discharge from hospital.

Responsibilities

In this role, you will,

  • Coordinate and facilitate safe, timely, and person-centred discharges from mental health inpatient settings, ensuring continuity of care and support in the community.
  • Undertake assessments of social care needs, including risk assessments, in line with the Care Act 2014 and Mental Health Act 1983.
  • Lead or contribute to discharge planning meetings, working collaboratively with multidisciplinary teams, including psychiatrists, nurses, occupational therapists, and community mental health services.
  • Develop and implement discharge plans that reflect the individual’s strengths, preferences, and recovery goals.
  • Ensure compliance with legal frameworks, including Section 117 aftercare, Mental Capacity Act 2005, and safeguarding legislation.
  • Advocate for service users, ensuring their rights, wishes, and best interests are upheld throughout the discharge process.
  • Liaise with community services, including housing providers, voluntary sector organisations, and primary care, to ensure appropriate support is in place post-discharge.
  • Monitor and review care arrangements, ensuring they remain appropriate and responsive to changing needs.
  • Maintain accurate and timely case records, reports, and documentation in accordance with professional and organisational standards.
  • Contribute to service development, quality assurance, and continuous improvement initiatives within the hospital discharge pathway.

To be successful in this role you must have, 

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
  • Registration: Must be registered with Social Work England.
  • Experience: Substantial experience working with vulnerable adults, including those with complex needs.
  • Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983, and safeguarding procedures.
  • Skills: Excellent assessment, analysis, and communication skills. Ability to work collaboratively in a multi-disciplinary setting.
  • Driving Licence: Full UK driving licence and access to a car is often required for community visits.
