Responsibilities

Deliver evidence-based nutrition education programs focused on chronic disease prevention and management.

Implement Food Is Medicine approaches such as medically tailored groceries, food pharmacies/pantries, produce prescription programs (FVRx), farmers market incentives, gleaning, and school-based strategies.
- →
Build and maintain partnerships with community organizations, schools, and healthcare providers to support program delivery and sustainability.
- →
Conduct outreach to increase awareness of and participation in nutrition education and Food Is Medicine programs.
- →
Coordinate with healthcare partners, including hospitals, primary care practices, and Federally Qualified Health Centers, to support referrals and enrollment.
- →
Adapt programming to meet local community needs while maintaining alignment with evidence-based models and program guidance.
- →
Maintain accurate records and complete required reporting and evaluation processes in alignment with CMS, Maine DHHS, and program partners.
- →
Participate in trainings, meetings, travel throughout the service area, and perform other duties as needed to support Healthy Lincoln County’s public health, prevention, and food access work.
Bachelor’s degree in nutrition, public health, health education, behavioral sciences, or a related field (or equivalent experience)
Experience building or maintaining community partnerships across diverse settings.
Ability to work independently while managing multiple responsibilities in a dynamic environment
Strong organizational, communication, and interpersonal skills
Proficiency with Microsoft Office, including Word and Excel
Flexibility to adapt to changing program needs and priorities
Valid driver’s license and reliable transportation is required
Ability to lift 30–40 lbs
