✓Utilize available resources to identify patients at high risk for rehospitalization and mortality and provide an extra layer of support.

✓Provide education and resources to patients, clinicians and community partners on available care options.

✓Support patients as the navigate the healthcare continuum.

✓Provide personalized advice based on the patient’s unique goals and care needs.

✓Provide guidance to patients during challenging situations and decision-making processes.

✓Assist with advance care planning tailored to a patient's wishes and values.

✓Educate and guide patients regarding healthcare systems, including home health, hospice, palliative care, hospitalizations, and skilled nursing or rehabilitation placement.

✓Maintain a strong sense of responsibility in responding to patients and clinicians, ensuring timely and effective support is provided as needed.

✓Ability to maintain careful records and document all patient/family interactions.

✓High school graduate or equivalent required. College degree preferred.

✓3 years’ experience in health care office, preferably in hospice and/or home health care.

✓Proficient computer skills required

✓Demonstrates excellence in customer service and time management

✓Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practices.

✓Demonstrates good communications, negotiation, and public relations skills.

✓Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.

✓Flexible schedule availability (based on business need)
