Utilize available resources to identify patients at high risk for rehospitalization and mortality and provide an extra layer of support.
Provide education and resources to patients, clinicians and community partners on available care options.
Support patients as the navigate the healthcare continuum.
Provide personalized advice based on the patient’s unique goals and care needs.
Provide guidance to patients during challenging situations and decision-making processes.
Assist with advance care planning tailored to a patient's wishes and values.
Educate and guide patients regarding healthcare systems, including home health, hospice, palliative care, hospitalizations, and skilled nursing or rehabilitation placement.
Maintain a strong sense of responsibility in responding to patients and clinicians, ensuring timely and effective support is provided as needed.
Ability to maintain careful records and document all patient/family interactions.
High school graduate or equivalent required. College degree preferred.
3 years’ experience in health care office, preferably in hospice and/or home health care.
Proficient computer skills required
Demonstrates excellence in customer service and time management
Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practices.
Demonstrates good communications, negotiation, and public relations skills.
Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
Flexible schedule availability (based on business need)
