Conduct initial and ongoing comprehensive assessments to determine home care needs, including physical and behavioral health evaluations.
Develop, implement and adjust care plans based on patient status and nursing diagnosis.
Administer prescribed medications and treatments, provide therapeutic and rehabilitative nursing interventions, and educate patients and families on health management, preventive care, and recovery strategies.
Collaborate with physicians, specialists, and community healthcare professionals to ensure seamless, holistic patient care.
Communicate updates regarding patient condition and secure necessary physician orders. Identify discharge planning needs and coordinate necessary follow-ups to promote patient well-being.
Prepare and maintain accurate clinical records, ensuring timely updates for physicians and compliance with healthcare regulations.
Review patient status and modify care documentation, providing consistent evaluations and care revisions as needed.
Participate in on-call duties as required, ensuring patients receive timely support and intervention.
Coordinate necessary medical equipment and services to enhance patient comfort and recovery.
Graduate of an accredited school of nursing and active state RN license.
Minimum of 1 year of recent (within the last 3 years) RN psychiatric experience.
Bachelor’s degree with 1 year of home health care experience strongly preferred.
Valid driver’s license and auto insurance.
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
Homecare Homebase (EMR) experience strongly preferred.
