✓Conduct initial and ongoing comprehensive assessments to determine home care needs, including physical and behavioral health evaluations.

✓Develop, implement and adjust care plans based on patient status and nursing diagnosis.

✓Administer prescribed medications and treatments, provide therapeutic and rehabilitative nursing interventions, and educate patients and families on health management, preventive care, and recovery strategies.

✓Collaborate with physicians, specialists, and community healthcare professionals to ensure seamless, holistic patient care.

✓Communicate updates regarding patient condition and secure necessary physician orders. Identify discharge planning needs and coordinate necessary follow-ups to promote patient well-being.

✓Prepare and maintain accurate clinical records, ensuring timely updates for physicians and compliance with healthcare regulations.

✓Review patient status and modify care documentation, providing consistent evaluations and care revisions as needed.

✓Participate in on-call duties as required, ensuring patients receive timely support and intervention.

✓Coordinate necessary medical equipment and services to enhance patient comfort and recovery.

✓Graduate of an accredited school of nursing and active state RN license.

✓Minimum of 1 year of recent (within the last 3 years) RN psychiatric experience.

✓Bachelor’s degree with 1 year of home health care experience strongly preferred.

✓Valid driver’s license and auto insurance.

✓Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.

✓Homecare Homebase (EMR) experience strongly preferred.
