✓Develops, implements and evaluates the orientation program for new organization personnel, and training programs to expand the capacity of all staff through in-services and continuing education programs.

✓Focus on PDGM (Patient Driven Groupings Model) and HVVBP education.

✓Implements practices Identified to improve clinical delivery, quality and service.

✓Assesses the learning needs and competence of staff to improve the level of performance, teaching skills, and quality of care, and assure compliance with accreditation standards

✓Develop curriculum and ability to precept.

✓Trends and analyzes utilization, outcomes, and quality of program data on an ongoing basis.

✓Works closely with all Branch Directors, Rehab Director and Quality Care Mangers to monitor educational standards and deficits.

✓Reviews clinical documentation and skills for ongoing learning needs.

✓Ensures new hire education and ongoing staff training is aligned with current clinical standards, guidelines, relevant statutes, rules, and regulations.

✓Provides education and support to the clinical staff in maintaining quality improvement outcomes and STAR ratings.

✓Participates in the development, implementation, and evaluation of best practices for Home Care/Hospice.

✓Assists Clinical Managers in evaluating the performance of new clinicians.

✓Regularly evaluates the appropriateness of the educational courses assigned to staff.

✓Facilitates learning experiences for staff through direct and indirect methods.

✓Ability to work and communicate via electronic communications, and possess knowledge of HCHB, SHP, Microsoft Word, Excel, and PowerPoint

✓Licensed / Registered Professional in the state of California.

✓Associate degree or higher from an accredited institution.

✓Recent clinical experience within Hospice

✓Working knowledge of HCHB (EMR)

✓Demonstrated ability to work effectively in a team.

✓Excellent observation, verbal, and written communication skills.
