Deliver comprehensive hospice services in accordance with the Nurse Practice Act and under a collaborating physician’s supervision.
Conduct patient evaluations, health assessments, diagnosis, and management of care plans, including prescribing medications and treatments.
Perform physical exams, face-to-face assessments, and hospice eligibility evaluations in consultation with the interdisciplinary team.
Collaborate with patients, families, and healthcare practitioners to coordinate care.
Utilize community resources to develop and direct services supporting patients and families at home and in the community.
Graduate of an accredited School of Nursing and Nurse Practitioner Program (or meet standards under California Code of Regulations Section 1484).
Current RN and NP licensure in the state; CPR certification required.
Master’s degree with at least one (1) year NP experience and one (1) year hospice or palliative care experience.
Certification by an approved advanced practice nursing certifying body; specialty certification (e.g., CHPN, Pediatrics, Geriatrics) preferred.
Strong assessment, communication, problem-solving, and nursing skills; able to meet physical demands of patient care.
Must have a valid driver’s license, reliable insured vehicle, and meet organization driving requirements.
