Lead Homecare CNA

Schedule: Full Time 40 hours/week | Monday-Friday
Department: Thome PACE | Jackson, MI

What You'll Do: 
As a Lead Homecare CNA at Thome PACE, you’ll play an important role in supporting participants who receive care in their homes while also helping guide and support the homecare CNA team. In this role, you’ll provide hands-on assistance with daily living needs such as personal care, mobility support, and other care plan-directed services, while helping ensure participants receive compassionate, consistent, and high-quality support.
You’ll also assist with coordinating daily care needs, mentoring and supporting CNAs, monitoring adherence to care plans, and maintaining clear communication with participants, families, and the care team. This role includes administrative support such as scheduling assistance, charting audits, and quality assurance tasks to help keep home care services running smoothly.
