The Quality & Population Health Manager is an experienced registered nurse. The role is responsible for leading quality improvement across patient safety and population health initiatives across the organization. This role oversees and actively participates in the development, implementation, and evaluation of programs designed to improve clinical outcomes, close gaps in care, enhance patient experience, and support regulatory and value-based care requirements. The Quality & Population Health Manager collaborates with operations, clinical, finance, marketing, and outreach teams to drive performance improvement, coordinate care management activities, and achieve the organization's quality goals. This position requires the ability to handle multiple and shifting priorities, work productively with minimal supervision, attention to detail, and excellent communication and presentation skills.

Essential Duties and Responsibilities

  • Supports and supervises RN Case Managers
  • Facilitate and coordinate monthly Quality Improvement and Quality Assurance Committee meetings.
  • Collaborate with quality improvement initiatives focused on patient outcomes, patient safety, patient experience, and operational performance.
  • Participates in the risk assessment and mitigation initiatives
  • Monitors, analyzes, and reports on the quality metrics updates
  • Oversees population health programs, case management activities, and chronic disease initiatives.
  • Actively collaborates with health plan quality teams and hospital case managers to plan and organize patient care transitions
  • Organizes and facilitates staff meetings, huddles and rounding
  • Works with the CNO and Director of Population Health to plan and manage the department’s operating budget and human resources
  • The quality manager will be involved in the coordination of services, assessment, monitoring and evaluation of the comprehensive health care needs of high-risk patients, ensuring the coordination of quality, cost-effective health care in a patient-centered environment
  • Collaborate to develop, revise policies, workflows, and staff education forums that support quality and population health initiatives.
  • Works closely with population health data analysis to capture, prioritize, and share actionable data that aligns with the organization's quality metrics and value-based goals.
  • Works closely with the Director of Population Health to identify key performance indicators.
  • Collaborate to execute and maintain granted support programs and projects.
  • Lead efforts and evidence-based validation to maintain national accreditations, i.e., PCMH
  • Lead and coordinate the completion of random quarterly departmental quality chart reviews and repository of results
  • Actively participates in health plan quality/population health meetings.
  • Coordinate and participate in quarterly on-site quality rounds

Requirements

  • Graduate of an accredited nursing school.
  • Licensure as a registered nurse in the state of Nevada
  • Knowledge of FQHC operations, HRSA requirements, NCQA standards, and UDS reporting measures.
  • Current CPR or BLS certification
  • Active Nevada Drivers License
  • Minimum of five years of experience in quality improvement and case management activities.
  • Working knowledge of Electronic Health Record systems
  • Master’s degree in nursing
  • Working knowledge of successful project management
  • Knowledge of the essential functions, practices and procedures of a medical clinic and office
  • Working knowledge of reporting and measurement of performance activities.
