New Vision Family Health Team is a multidisciplinary team of healthcare professionals dedicated to providing high-quality, patient-centred primary care in a caring, coordinated, and responsive environment. Located in the Waterloo Region, we provide comprehensive primary care services to more than 30,000 patients. Position Summary

As a Registered Dietitian (RD) with New Vision Family Health Team, you will work both independently and collaboratively with physicians and other healthcare professionals to provide nutrition care, health promotion, and chronic disease management services. As an integral member of our interdisciplinary team, you will support patients in achieving their health goals while contributing to the delivery of effective, timely, and patient-centred care.


Key Responsibilities

Patient Care


  • Practice in accordance with the standards and guidelines of the College of Dietitians of Ontario.
  • Screen, prioritize and process patient referrals using information from physicians, laboratory reports, and health records.
  • Provide nutrition assessments for patients of all ages, considering their level of knowledge, readiness to change, and socio-demographic background.
  • Develop individualized nutrition care plans in collaboration with patients and the healthcare team.
  • Provide nutrition counselling and education, including follow-up assessments and support for behaviour change.
  • Ensure complete, accurate, and timely documentation of program activities and electronic medical records.


Education & Health Promotion


  • Deliver group health education sessions on nutrition-related topics using established teaching and learning principles and lifestyle counselling.
  • Present clinical practice guideline updates to healthcare providers and clinical partners.
  • Assist in the development of patient programs by identifying priorities, target dates, and implementation strategies.


Collaboration & Professional Practice


  • Build and maintain relationships with community partners.
  • Work effectively as part of an interdisciplinary team


Qualifications


  • Undergraduate degree in Food and Nutrition from a recognized university or equivalent.
  • Completion of a Dietitians of Canada-accredited Dietetic Internship, equivalent approved experience, or graduate degree in Community Nutrition.
  • Registered and in good standing with the College of Dietitians of Ontario.
  • Demonstrated experience in a community or primary care setting.
  • Experience with case management and case coordination involving members of the interdisciplinary team and community services is preferred.
  • Ability to work independently and collaboratively within a team environment.
  • Proficiency with computers and electronic medical record systems.
  • Ability to work on site.
  • Successful completion of a Criminal Record Check with Vulnerable Sector Screening.
  • Completion of health screening requirements (MMR, Tdap, Varicella, and Hepatitis B).
  • Flexibility to work Mondays, Wednesdays, Thursdays or Fridays and availability to work at least one evening per week (Wednesday or Thursday).


Compensation & Benefits


  • Hourly wage ($38.54 - $40.48), commensurate with experience
  • Healthcare of Ontario Pension Plan (HOOPP)
