Responsibilities

  • Complete comprehensive nursing admission assessments, including collection of medical history, medication history, current symptoms, risk factors, and relevant clinical information to support safe patient placement and treatment planning.
  • Perform nursing assessments and identify medical concerns requiring provider evaluation, intervention, or additional monitoring.
  • Complete admission documentation accurately and timely within the electronic medical record system (KIPU), including assessments, medication reconciliation, nursing notes, and required admission forms.
  • Provide admission orientation, education, and information regarding program expectations, medications, safety procedures, and treatment processes to patients and families as appropriate.
  • Communicate significant clinical observations, changes in patient condition, and admission concerns to providers and the multidisciplinary treatment team.
  • Collaborate with Admissions, Medical Providers, Counseling, Utilization Review, and Operations teams to facilitate a safe and efficient admission process.
  • Review incoming clinical information and communicate relevant findings to appropriate team members to support continuity of care.
  • Obtain vital signs, monitor symptoms, assess withdrawal risks, and document findings in the patient record.
  • Demonstrate knowledge and adherence to withdrawal management protocols and complete appropriate assessments and interventions as indicated.
  • Administer medications safely and accurately, including patient education regarding medication purpose, administration, and potential side effects.
  • Monitor patients on an ongoing basis to assess medical needs and facilitate appropriate treatment interventions.
  • Provide routine nursing care and clinical support as assigned, including medication administration, patient monitoring, education, and completion of nursing responsibilities.
  • Utilize appropriate de-escalation techniques when interacting with patients experiencing emotional distress or behavioral concerns to maintain safety for patients, staff, and others.
  • Maintain professional boundaries and therapeutic relationships with patients at all times.
  • Participate as an active member of the multidisciplinary treatment team, contributing clinical information to treatment planning and discharge planning.
  • Communicate appropriately with outside providers, pharmacies, agencies, and other involved parties as needed while maintaining confidentiality.
  • Complete individualized nursing care plans and update documentation based on patient needs.
  • Document nursing assessments, interventions, patient response, progress notes, and other required clinical documentation in a timely and accurate manner.
  • Participate in quality assurance, compliance, and performance improvement activities as assigned.
  • Assist with providing medical information to Utilization Review and managed care representatives to support coordination of services and reimbursement.
  • Maintain a clean, orderly, and safe treatment environment and report safety concerns, maintenance needs, or repairs appropriately.
  • Complete required daily operational tasks, including temperature logs, sanitation logs, and other assigned facility checklists.
  • Complete orientation and competency validation for CLIA-waived testing as required.
  • Comply with all applicable laws, regulations, ethical standards, company policies, and scope of nursing practice.
  • Maintain confidentiality in accordance with HIPAA and established organizational policies and standards of care.
  • Complete other clinically appropriate duties and special projects as assigned.

 

  • Interact routinely with patients, observe behaviors and communicate significant observations to multidisciplinary team as necessary
  • Consult with the multidisciplinary treatment team in the development of treatment plans and discharge planning to strive for excellent patient care
  • Participate in the maintenance of a clean, orderly and safe environment, and report needs for repairs and maintenance to appropriate staff
  • Obtain vital signs, monitor symptoms and document in the patient record
  • Comply with all laws, regulations, and scope of licensure
  • Monitor patients on an ongoing basis to continually assess needs and facilitate medical treatment and interventions
  • Document medical assessment, diagnosis, progress notes, roundings, and other treatment planning items in a timely manner to assure comprehensive and quality patient care

 

Requirements

  • Current, unencumbered, active license to practice as a Registered Nurse in the state where services are rendered
  • Must be at least twenty-one (21) years of age
  • Current and nationally accredited CPR/First Aid/AED certification
  • Knowledge of computer and electronic medical records systems (KIPU preferred)
  • Enthusiasm, passion for working with people, and an internal drive to improve the lives of individuals in our care
  • Must maintain applicable state licensure requirements throughout duration of employment
  • Ability to work with a diverse population along with interpersonal skills and knowledge required for treating patients in area of assigned specialty
  • Demonstrated ability to hold strong personal boundaries and ability to build rapport with patients
  • Must understand and adhere to the ethical standards of the respective licensure governing board
