Family Medicine at South Waterfront is a high-volume primary care clinic located in the Center for Health & Healing 1 building. The clinic provides comprehensive, lifespan care to patients across all stages of life through in-person, virtual, and telephone visits.
Services include general family medicine, transgender health care, maternity and prenatal care (including deliveries), pediatric care, behavioral health and counseling, sports medicine, medication-assisted treatment (MAT), podiatry and foot care, acupuncture, minor procedures, and preventive health services.
Clinical care includes medication administration (oral and injectable), telephone and in-clinic triage, care coordination and management, patient education, complex wound care, pain management, and behavioral health support, including crisis intervention.

Care is delivered to patients with a wide range of needs—from routine preventive care to complex, chronic conditions, often impacted by social determinants of health (SDOH).
Responsibilities
~1 min readRegistered Nurses are integral to care delivery and perform clinical triage and patient advice, care coordination and transitions of care, chronic disease management, preventive care and population health work, visit-based nursing care, patient education and self-management support, and medication administration and management.
- →Experience in independent RN triage via phone and in-basket messaging utilizing protocols
- →Conducts RN led visits within scope i.e. iv placements, medication administration, chronic disease education, and wound care
- →Experience in longitudinal care management and care coordination
Requirements
~1 min read- BSN Graduates: Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date.
- ADN Graduates: Associate Degree in Nursing from an accredited program 30 days before the start date.
- Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire
- Current, unencumbered Oregon State Registered Nurse License
- Active Washington RN License within 6 months of hire
- BLS from AHA required. New hires will be enrolled and required to complete during orientation.
- Must be able to perform the essential functions of the position with or without accommodation
Requirements

- Current Basic Life Support (BLS) issued by American Heart Association (AHA)
Length of Orientation – New Graduate Nurse
- External candidates: OHSU & Nursing New Employee Orientation (NEO) for about a week.
- Either Transition to Practice (TTP) Program Residency Learning Pathway or Orientation experience for about 10-26 weeks depending on the care area. An employment service agreement may apply.
Length of Orientation – Experienced Nurse
- External candidates: OHSU & Nursing New Employee Orientation (NEO) for about a week.
- Either Transition to Practice (TTP) Program Specialty Fellowship/Fellowship Learning Pathway or Orientation experience for 3 days-26 weeks depending on the care area. An employment service agreement may apply.
