Home Care Manager

• →Must have reliable transportation as traveling valley wide is required
• →Initial Client Intake visits (in the client’s home)
• →Conduct client-in-home visit reviews (5-days, 30-days, and 90-days)
• →Create Care Plans for all clients
• →Document any issues and/or positive information in the Client Logging and/or Caregiver Logging system in CL.
• →Updates information as needed in a timely manner within our EMR system
• →Update Excel Workbook trackers – Client related
• →Report any concerns to the MCO CM and the Office immediately via email
• →Any other duties as deemed necessary by the Director
• →May have to provide care for clients at any given time due to call off’s if not able to restaff with a caregiver
• →Will be compensated weekly for “on-call – after-hours” emergency calls

• High school graduate or GED preferred
• Excellent computer skills and must be detailed oriented/accurate
• Must have good interpersonal communication, time management, and problem-solving skills.
• Must pass a background check as well as the ability to obtain Finger Print Clearance Card

• ?Bilingual is a plus (English and Spanish)
