JOB SUMMARY
To perform a variety of skilled tasks in the implementation and operation of the Nurse-Family Partnership program, which is a highly refined and successful approach to the long-established service strategy of nurse home visiting.

DISTINGUISHING CHARACTERISTICS
Nurse Home Visitors must have a great deal of personal maturity and a strong desire to do home-based preventive intervention with young families that requires the development of a therapeutic relationship with each parent in the program. It is more than a teaching role, and nurses in this program will be required to become expert in nurturing the development of young parents across several domains of functioning, including health.

SUPERVISION RECEIVED AND EXERCISED

Receives general supervision from the NFP Nurse Supervisor, Director of Health Services and/or the Assistant Director of Health Services.

EXAMPLES OF IMPORTANT RESPONSIBILITIES AND DUTIES - Essential and other important duties and responsibilities may include, but are not limited to, the following:

Target low-income, first-time mothers (having no previous live births) for enrollment in the Nurse-Family Partnership program.

Ensure that mothers are enrolled in the program by the 16th week of pregnancy, preferably earlier, and not after the 28th week of pregnancy; and continuing visits through the first two years of the child's life.

Follow in-depth program guidelines that focus on the mother's personal health, quality care-giving for the child and the mother's own development.

Follow a visit schedule matched to the developmental stages of pregnancy and early childhood and adjusted to meet each family's needs.

Carry a caseload of no more than 25 active families.

Maintain a flexible schedule that allows for evening or weekend visitation if necessary.
Be involved in the mother's support system, including family members, the father when appropriate, and friends.

Participate in helping families learn how to access and use other health and human services they may need.

Meet weekly with the NFP Nurse Supervisor for case conferencing and professional development.

Utilize the Nurse Family Partnership data collection and reporting system that has been designed for the program to keep track of family characteristics, program operation and progress toward accomplishing objectives to achieve excellence in program performance.

Attend required workshops, conferences and in-service training sessions in support of the Nurse-Family Partnership program that will include:
 
  • Unit 1, Fundamentals of NFP Nursing Practice
  • Unit 2, the first face to face session in Denver, CO
  • Unit 3, the second face to face session in Denver, CO

Maintain and update a variety of records, files and reports pertaining to the Nurse-Family Partnership program.

Perform other related duties as assigned.

QUALIFICATIONS

Knowledge of:
Nursing principles, practices, methods and techniques of the assigned program area. Current trends and developments in the assigned program area.

Public health program development principles and techniques.

Ability to:
Maintain an appropriate level of current knowledge and understanding regarding assigned nursing and program responsibilities.

Establish and maintain cooperative working relationships with those contacted in the course of work.

Communicate effectively, both orally and in writing.

Work independently in the absence of supervision.

EXPERIENCE AND TRAINING GUIDELINES: Any combination equivalent to experience and training that could likely provide the required knowledge and abilities is qualifying. A typical way to obtain knowledge and abilities would be:

Experience:
Two (2) years of experience in nursing; and/or some experience in obstetrics, maternal and child health, community health nursing or public health.

Training:
A Bachelor of Science Degree in Nursing from an accredited college or university is required.

License or Certificate:
Possession of, or ability to obtain, an appropriate Texas driver's license.

Possession of a valid license to practice professional nursing in the State of Texas.
