The Wound Care Preventionist is responsible for conducting regular, proactive skin and wound assessments to prevent the development of pressure injuries and other wound-related complications. This clinical role supports overall patient safety and outcomes by identifying early risk factors and ensuring proper preventative measures are implemented in coordination with the clinical team.

What You’ll Do

• Perform scheduled skin integrity and wound prevention assessments on at-risk patients/residents
• Conduct thorough assessments to identify early signs of skin breakdown and collaborate with nursing staff to initiate appropriate interventions
• Develop and implement individualized, evidence-based treatment plans to support wound prevention.
• Partner with facility healthcare teams to coordinate care plans and treatment updates.
• Document all prevention visits, observations, and recommendations accurately in the electronic medical record (EMR)
• Monitor the effectiveness of preventive measures and report any changes or concerns to the wound care nurse or provider
• Ensure consistent use of appropriate support surfaces, positioning techniques, and barrier products as needed
• Track and follow up on high-risk individuals to ensure continuity of preventive care
• Communicate clearly and professionally with clinical staff to reinforce skin integrity protocols
• Maintain compliance with facility policies, clinical standards, and regulatory guidelines
• Ensure continuity of care through consistent communication and collaboration.
• Multi-Facility Travel & Scheduling visiting multiple facilities weekly.
• Ensure timely and efficient care delivery with professionalism and compassion.

What We’re Looking For

• Valid Nurse Practitioner license (State-specific)
• ANCC or AANP Board Certification
• 1+ years of experience in advanced wound care highly preferred
  
• Wound care certification (CWOCN, CWS, WCC, WOCN) is a plus
• Reliable transportation and willingness to travel consistently
