• →Delivering personalized high-quality care to seniors in their homes, assisted living, hospice, and nursing facilities. 
• →Delivering dementia-informed, relationship-based care across the continuum of care. 
• →Conducting comprehensive assessments and developing tailored care plans for older adults. 
• →Managing chronic conditions and performing care with skill and empathy for patients and their families. 
• →Collaborating with cross-functional partners – including palliative, psych, and neuro – to deliver coordinated, whole-person care. 
• →Building trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals. 
• →Maintaining consistency in documentation, follow-through, and patient communication. 

• Active, unencumbered NP or PA license in the state you will be practicing. 
• Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred). 
• Compassionate, patient-centered approach and strong clinical judgment. 
• Valid driver’s license and reliable transportation.
