We’re hiring a Licensed Social Worker (LMSW or LCSW) – a compassionate and skilled professional who provides emotional support, counseling, and resource coordination for patients and families throughout the hospice journey. This role ensures holistic care and helps families navigate the challenges of end-of-life planning with dignity and compassion.

Responsibilities
~1 min read- →Conduct assessments, develop care plans, and document bereavement activities
- →Design and distribute educational materials for families receiving bereavement services
- →Develop programs on grief and loss for families and staff
- →Evaluate the bereavement program quarterly and annually
- →Provide community consultation and education on hospice services
- →Maintain ongoing education through academic studies and workshops
- →Attend in-services, staff meetings, and case conferences
- →Report patient condition changes to the RN Case Manager
- →Observe and address safety hazards in patient and organizational environments
- →Assist the health team in understanding emotional factors related to terminal illness
- →Assess suicide risk, mental capacity, and elder/dependent adult abuse
- →Aid in advance directives, mortuary arrangements, and financial planning
- →Provide information and referrals to community resources and legal entities
- →Advise and counsel staff, patients, families, or agencies in emergency situations
Requirements
~1 min read- Master’s degree in Social Work required
- Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) required
- Documented evidence of training in grief and loss counseling
- Valid driver’s license and clean driving record required
- Must comply with all state and federal driving regulations
- Reliable transportation and willingness to travel to patient locations
- Comfortable working in an environment with pets
