About SOAHAC
Southwest Ontario Aboriginal Health Access Centre (SOAHAC) is a diverse, dynamic, multi-service Indigenous health and wellness organization. We provide wholistic, culturally grounded care by integrating traditional Indigenous healing and western health practices to support balance and wellness for individuals, families and communities.
SOAHAC’s London site serves First Nations (on- and off-reserve), Inuit and Métis peoples in and around the City of London. SOAHAC provides culturally safe, trauma-informed, relationship-based care that supports balance across the physical, mental, emotional and spiritual aspects of wellness. We work in partnership with Indigenous communities, Elders, Traditional Healers, Knowledge Keepers and interdisciplinary care teams to strengthen wellness, belonging and self-determination.
Position Summary
Reporting to the Senior Manager, Client Care, the Integrated Care Manager provides leadership within an Indigenous led, wholistic model of care.
This role supports day-to-day operations, team development and coordination of interdisciplinary health and wellness services at SOAHAC’s London site. The Integrated Care Manager works closely with Traditional Healing teams, physicians, nurse practitioners, mental health and addictions workers, allied health professionals and community partners to support integrated care pathways grounded in cultural safety, relational practice and Indigenous wellness approaches.
The successful candidate will demonstrate strong relational leadership and the ability to support collaborative practice within a multicultural environment serving Indigenous communities. This role requires a balance of operational leadership, people leadership, cultural humility and systems coordination to ensure services remain accessible, responsive, trauma-informed and aligned with SOAHAC’s Indigenous-led integrated care framework.
The Integrated Care Manager contributes to building a respectful organizational culture that honours Indigenous knowledge systems, supports staff wellness and accountability and strengthens wholistic care outcomes for clients, families and communities.
Responsibilities
Provide operational and relational leadership for an inter-professional, multi-cultural health care and wellness team within an Indigenous led, wholistic integrated care model.
Support respectful and effective integration across Traditional Healing, Primary Care, Mental Health and Addictions, Social Work, Allied Health, community wellness and outreach services
Foster strong interdisciplinary relationships grounded in cultural safety, humility, accountability and Two-Eyed Seeing principles
Support staff in delivering trauma-informed, relationship-based care that honours Indigenous identities, lived experiences and healing journeys
Work collaboratively with Traditional Healers, Elders, Knowledge Keepers and clinical teams to support culturally grounded wellness planning and integrated client care
Support workforce development, onboarding, mentorship, coaching and performance management in alignment with SOAHAC’s values and leadership expectations
Promote healthy team functioning through respectful communication, collaborative problem-solving and restorative approaches to conflict resolution
Support operational planning, staffing coordination, scheduling practices and service accessibility to ensure timely and equitable care
Monitor service trends and participate in quality improvement activities that strengthen client experience, integrated care outcomes and culturally grounded practice
Support organizational learning and continuous improvement related to Indigenous cultural safety, anti-racism and integrated wholistic care
Ensure organizational policies, health and safety practices, privacy requirements and professional standards are maintained
Participate in community relationship-building and organizational initiatives that advance Indigenous health equity and self-determination
Qualifications & Experience
Post-secondary education in healthcare, leadership, public administration, business, or a related field (or equivalent combination of education and experience)
Minimum five (5) years of progressive leadership experience within community health, Indigenous health, primary care, mental health, or integrated wellness environments
Demonstrated ability to lead interdisciplinary teams within collaborative and relationship-based care environments
Strong understanding of primary care operations, service access and integrated care delivery models
Experience with access improvement, panel management, or primary care scheduling models is considered an asset
Experience working respectfully with First Nations, Inuit and Métis peoples, communities, organizations and Traditional Healing teams
Understanding of Indigenous determinants of health, intergenerational trauma, colonial impacts and culturally safe care practices
Knowledge of Indigenous wholistic wellness approaches and experience supporting integrated care models that include Traditional Healing and western clinical services
Demonstrated commitment to cultural humility, reflective practice and ongoing learning
Strong relational leadership, communication, facilitation and conflict resolution skills.
Support psychologically safe, culturally respectful and wellness-oriented team environments.
Experience managing competing priorities and supporting organizational change, team wellness and collaborative problem-solving
Proficiency with Microsoft Office and electronic medical record systems
Valid driver’s license, reliable vehicle and ability to travel within the London region, as well as occasionally to other SOAHAC sites or provincially
Satisfactory and current Police Record Check is required
Up-to-date immunization documentation in accordance with organizational requirements
Leaders at SOAHAC are expected to model relational accountability, humility, collaboration and respect in all interactions with staff, clients, communities and partners.
