Position Summary: TAN Healthcare is seeking a compassionate, mission-driven Physician (MD/DO) specializing in Family Medicine or General Practice to join our growing integrated healthcare team serving Southeast Texas communities.

This position offers an opportunity to practice meaningful, relationship-based medicine in a community-centered healthcare environment focused on improving access, stability, and long-term health outcomes for medically underserved populations.

The Physician will provide comprehensive primary and preventive care services across the lifespan while working collaboratively within an interdisciplinary care model that includes behavioral health, chronic disease management, transportation services, outreach and education, and care coordination support.

Essential Job Responsibilities

  • Provide comprehensive primary care services including diagnosis, treatment, and management of acute and chronic medical conditions
  • Perform routine physical exams, wellness visits, preventive screenings, and health maintenance services
  • Diagnose and manage chronic diseases including hypertension, diabetes, asthma, and other common primary care conditions
  • Order, interpret, and evaluate laboratory tests, imaging studies, and diagnostic procedures
  • Prescribe medications and treatment plans as appropriate
  • Provide patient education focused on prevention, medication adherence, chronic disease management, and healthy lifestyle practices
  • Coordinate care with specialists, behavioral health providers, care managers, outreach staff, and other interdisciplinary team members
  • Participate in integrated care planning to improve patient outcomes and continuity of care
  • Maintain accurate, timely, and compliant documentation within the electronic medical record system
  • Participate in quality improvement initiatives, clinical meetings, peer collaboration, and organizational activities
  • Support TAN Healthcare’s mission of providing accessible healthcare services regardless of a patient’s ability to pay
  • Provide care at both TAN Healthcare clinic locations as scheduled
  • Participate in limited shared call rotation as applicable
  • Follow all organizational policies, clinical protocols, compliance standards, and regulatory requirements
  • Perform other duties as assigned

 

Education, Qualifications, and Experience

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school
  • Board Certified or Board Eligible in Family Medicine or General Practice
  • Current unrestricted Texas Medical License in good standing
  • Current DEA registration
  • Basic Life Support (BLS) certification required
  • Experience in family medicine, primary care, community health, rural health, or federally qualified health center (FQHC) settings preferred
  • Commitment to serving diverse and medically underserved populations
  • Ability to work effectively within a team-based integrated healthcare environment

  • Strong clinical assessment, diagnostic, and treatment planning skills
  • Commitment to preventive medicine and chronic disease management
  • Ability to build positive patient relationships and provide compassionate care
  • Strong communication and patient education abilities
  • Ability to work collaboratively within interdisciplinary care teams
  • Understanding of electronic medical record systems and clinical documentation standards
  • Ability to manage competing priorities in a fast-paced healthcare environment

Work Environment / Schedule

  • Clinic-based healthcare environment
  • Full-time position
  • Work schedule may include occasional extended clinic hours based on organizational needs
  • Position requires travel between TAN Healthcare clinic locations in Beaumont and Orange, Texas
  • Shared call responsibilities may apply based on provider scheduling structure
